The treatment of patients with chronic I.T.P., especially who fail to respond to corticosteriods and splenectomy (refractory I.T.P.) is often difficult since these patients tend to be resistant to many forms therapy. On the other hand, the long-term treatment with the aggressive (invasive) drugs is limited due to extremely high rate of side effects, complications and low rate of positive long-term outcome. Moreover, these medicines are very costly. Based on our extensive clinical experience in the management of chronic I.T.P., patients who have serious hemorrhage including frequent epistaxis requiring intervention, gross hematuria or other bleeding causes a decline in hemoglobin concentration. The relevant therapy should be the one which is safe, non-complicated but which effectively controls bleeding symptoms and raises platelet counts to higher than critical levels (50,000/ml).
